Step One

Complete our Interest Form to let us know how we can partner with you and to schedule a visit.



PLEASE NOTE: We are currently not scheduling tours or enrolling new children. Please fill out the form if you are interested in being put on our waiting list. Melinda Neuenfeldt, the Director, will be in touch with you shortly.


Step Two

Complete and submit the following forms.



Tell us about your needs, such as how many days care is needed, age of the child, start date, and more.  Complete and return to us via email, childcare@trinityfreistadt.com, or drop off in person.



The Tuition Express Authorization form allows us to reserve your child’s seat and collect tuition and other charges from your designated bank account (checking or savings) or credit/debit card (VISA or MC). Complete and return to us via email, childcare@trinityfreistadt.com, or drop off in person.



Continue the registration process by submitting information through MyProcare.  We use MyProcare to manage daily scheduling and billing.


Step Three

The following forms will need to be completed and returned to us prior to the start of your child’s first day.



Parental consent for several policies necessary to our partnership.



Let us know who to call in case we cannot reach you when your child is in our care and who is authorized to pick up your child.  Our Center staff will require ID in order to pick up your child.



This online form only needs to be completed by parents of children 6 weeks through 1 year of age.



A health report for each child must be submitted upon enrollment. These records are kept on permanent file at the center.



An immunization record for each child must be submitted upon enrollment. These records are kept on permanent file at the center and must be updated as your child receives immunizations.



Authorize us to serve your child (one year and older) snacks, hot lunch, and milk.

Hot lunch is available during Trinity’s School year only.



IMPORTANT: Complete this form if your child has an allergy or other medical concerns we need to be aware of.
