Step Two

Complete and submit the following forms.



The Intent to Enroll form provides valuable information such as how many days care is needed, age of the child, start date, and more.  Complete and return to us via email or drop off in person.



The Tuition Express Authorization form allows us to reserve your child’s seat and collect tuition and other charges from your designated bank account (checking or savings) or credit/debit card (VISA or MC). Complete and return to us via email or drop off in person. Please note we do not have access to your personal account information once it is entered into our system. The paper form is destroyed immediately upon entering and verification.



Continue the registration process by submitting information through MyProcare.


Step Three

The following forms will need to be completed and returned to us prior to the start of your child’s first day.



Acknowledgment and consent of each of the following are maintained on file.



Let us know who to call in case we cannot reach the parental guardians and who is authorized to pick up your child. Please note that Center staff will require ID in order to pick up your child.



‘Tell Us About Your Infant’s Daily Routine’ is an online form that only needs to be completed by parents of children 6 weeks through 1 year of age.



A health report for each child must be submitted upon enrollment. These records are kept on permanent file at the center.



An immunization record for each child must be submitted upon enrollment. These records are kept on permanent file at the center and must be updated as your child receives immunizations.



Fill this form out to authorize us to serve your child (one year and older) snacks, hot lunch, and milk.



IMPORTANT: This form will need to be completed if your child has an allergy or other medical concerns we should be aware of.
